The First Impressions clerk position is located in the patient registration reception area. The purpose of this position is to assure that each patient contacting the Wind River Family and Community Health Care (WRFCHC) has access to healthcare. The First Impressions clerk is responsible for obtaining and recording comprehensive and accurate data. This includes interviewing patients, establishing a health record for new patients, and updating the demographic information for patients at each clinic visit. The First Impressions clerk will be the first contact the patient makes when entering the facility for services. The First Impressions clerk will greet all patients in a friendly and courteous manner.
When opening the patient registration reception area, the patient registration staff should be completely prepared to receive patients when the facility is scheduled to open. This may require employees to report to work 5-10 minutes early if necessary.
There are numerous types of appointments available and the patient registration staff must be familiar with the standards of procedure in general facility information, appointment times, length of appointments, etc.
When the patient arrives, the First Impressions Clerk will verify the patient’s identity with 2 patient identifiers (i.e., name and date of birth). The First Impressions Clerk will check the appointment list to verify the appointment.
Updating Patient Demographics:
The First Impressions Clerk will update patient demographics at each patient encounter.
Patient demographics include:
Mailing and physical addresses
Date mailing address was changed
Emergency contact information
Medicare, Medicaid, Railroad Retirement, Private Insurance, Veterans Administration, Pregnant Women, Children’s' Health Services information
Missing documents (CIB, custody order, divorce decree, descendancy, marriage license, etc.)
Entering Third Party Information
Worker’s Compensation, Veteran Affairs (VA), etc.
The clerk ensures required documents, such as Certificate of Indian Blood (CIB), Tribal ID cards, descendancy forms, birth certificates, marriage license, divorce decrees, court custody orders, affidavit of paternity, and other legal documents are obtained from the patient, parent or legal guardian for eligibility of services.
The First Impressions Clerk will perform the following benefits coordination activities:
Work closely with Benefits Coordinator/PRC on alternate resources.
Inform patients of different programs available; such as:
Pregnant Women's Program
Medicare (Parts A & B)
Verifies alternate resources through:
Social Security Administration
Workman's Compensation Office
Knowledge, Skills, and Abilities:
Knowledge of patient registration concepts, practice, policies, requirements and techniques
Ability to schedule appointments
Ability to provide customer service
Oral communication skills
Knowledge of patient confidentiality
Knowledge of the Health Insurance Portability and Accountability Act (HIPAA)
Knowledge of The Privacy Act of 1974 regulations and requirements
Ability to initiate and follow through with assignments using policies and procedures
Knowledge of third party payer requirements
Ability to communicate with third party payers, tribal programs, and other healthcare entities
Maintains a good working relationship within the patient registration work area and with other department staff
The supervisor defines the overall goals and priorities and is available for guidance with unusual problems.
The supervisor relies upon the First Impressions clerk for the knowledge, skills, and abilities to independently perform his/her assignments.
The supervisor periodically reviews the work for results, technical accuracy and conformity to Health Information Management policy and regulatory requirements.
The supervisor places considerable reliance upon the clerk’s knowledge of patient registration standard reviews and reviews work for results, technical accuracy, and conformity to First Impressions policy and standards.
Guidelines include numerous facility policies, Federal and State laws as applicable and Centers for Medicare and Medicaid (CMS) requirements. Guidelines are general and do not cover all areas encountered in work performed. Interpretation of vague or unclear situations in which no precedent has been established is referred to the supervisor.
The clerk makes decisions regarding collecting, identifying, and recording patient demographic and eligibility information. It is important that the registration information is accurate and current.
Scope and Effect:
Work involves direct contact and care of the patient. Accessing patient health information for job duties.
Personal Contacts/Purpose Of Contacts:
Contacts are with physicians, nursing staff, business office staff, employees within the immediate organization or work unit.
The work is primarily sedentary. There may be some walking or carrying light items such as manuals or files. Good eye/hand coordination is required.
The work environment involves risks and discomforts of a patient care setting including exposure to communicable diseases, working with office machines and computers. The demand of computer terminals and keyboards for long periods of time may cause eye, shoulder, and wrist strain. Work is performed in a smoke-free office setting. There is adequate light, heat, and ventilation in the work area.
Other Significant Factors:
It is of high importance to provide good customer service to patients, co-workers, and other departmental staff. Maintain professionalism in the work environment.
Other duties as assigned within the department.
The incumbent must maintain strict confidentiality and high ethical standards in performing the position. The incumbent needs to be respectful, possess cultural awareness and sensitivity, be flexible, and demonstrate sound work ethics.
